INSTRUCTIONS 8D纠正措施说明
1.Team Members: Establish a small group of people with the process/product knowledge, allocated time, skill and authority in the required technical disciplines to solve the problem and implement corrective actions. Identify the Team Leader, who is responsible for directing the team and ensuring the 8D process.
团队人员:成立一个小组,小组成员都具备工艺/产品知识 能按照要求的技术规定,确定时间、技术和职权,解决问题并实施纠正措施。指定小组组长,组长须负责管理团队并确保8D过程。
2.Problem Description: Specify the internal/external customer problem by identifying “what is wrong with what’, and describing the problem in quantifiable terms which will answer the questions: Who?, What?, When?, Where?, How?, and How Many?
问题描述:通过发现“什么零件的什么位置错了什么”,以及以量化的形式回答“谁”,“什么”,“什么时间”“哪儿”“怎么干的”及“数量多少”这些问题来说明问题的方式,细化内部/外部客户问题。
3.Containment Action(s): Define and implement interim containment actions to isolate the effects of the problem from any internal/external customer until permanent corrective action is implemented. Verify effectiveness of containment action.
抑制措施:在永久性纠正预防措施实施前,定义并实施临时抑制措施,隔离对内部/外部客户造成的影响。验证抑制措施的有效性。
4.Root Cau(s): Identify all possible caus which could explain why the problem occurred (what it is and what it is not). Identify the most probable root cau(s) by testing each possible cau against the problem description and verify the by testing, obrvation, data collection, etc.
根本原因:查出能够解释问题为什么发生的所有的原因,(哪些是及哪些不是)。通过按照问题描述对每一个可能原因进行测试,查出最为可能的根本原因。然后通过测试、观察、数据收集等对这些根本原因进行验证。
5.Corrective Action(s): Choo the permanent corrective action confirming that the le
cted action will resolve the problem for the customer and will not cau undesirable side effects. Define contingency actions, if necessary, bad on risk asssment.
5a Identify any error-proofing methods ud.
5b. Address other areas which may be impacted.
纠正措施:选择永久性纠正措施,确认所选的措施会帮助客户解决问题并且不会导致不良的负面影响。如有必要,基于机遇风险评估制定系列应急措施:
a.确认任何可采用的防错方法。
b.说明其他可能受影响的方面。
6.Implement Corrective Action: Establish a plan to implement the permanent corrective action. Choo ongoing control to ensure the root cau is elimina0ted. Monitor the effects and implement contingency actions if necessary. Indicate a date that the corrective action is implemented, as well as a date that the corrective action was validated。
实施纠正预防措施:制定一个实李记白水羊头
施长久措施的计划。选择持续控制,以确保根本原因的消除。监督措施实施效果,必要时采取系列应急措施。标明纠正预防措施实施日期以及有效日期。
7.Actions to Prevent Recurrence: Modify the necessary systems, practices, and procedures to prevent recurrence of this and similar problems. Identify opportunities for improvement and establish a process improvement initiative (this could result in development of another team).
防止再次发生的措施:修改配套体系,工作惯例以及规程以防止这一问题与类似问题重复发生。寻找改进机会与创建工艺改进主动性。(可能使另一小组得到改进)。
8.Congratulate Team or Individual Results: The management owner shall recognize the collective efforts of the team or individual solving the problem and show appreciation for contributions. This may include involving other management personnel to determine appropriate recognition.
庆祝小组或个人获得的成果:管理负责人应承认小组或个人对解决问题的所付出的集体努力,并对其所作的贡献表示感激。这也可以让其他管理人员参与,以确定给予合适的奖励。
