长程脑电图在重症脑血管病演变过程中对脑功能的评价
摘要
目的:脑电图(electroencephalogram,EEG)是反映脑功能变化的监测技术之一,具有可在床旁进行、操作简便的优点,它对脑血管病患者的治疗及预后提供可靠资料。EEG监测对重症脑血管病患者脑功能评价有重要临床意义。本研究拟对重症脑血管病患者进行长程EEG监测,观察分析在重症脑血管病演变过程中EEG 的变化规律,探讨其对重症脑血管病患者脑功能评价,旨在为临床评估重症脑血管疾病患者脑功能提供依据。
方法:选自2007-01-01至2009-12-31我院神经内科收治的发病72小时以内、格拉斯哥昏迷计分(glasgow coma score,GCS)<8分的重症脑血管病患者180例,采用Solar2000N神经中央监护仪对所有患者进行常规长程脑电监测,并给予相应的EEG分级和GCS评分,统计所有患者的生存和死亡人数。比较脑梗死组和脑出血组患者EEG各级构成;分析所有患者EEG构型的特点;分析EEG分级与脑疝发生率、GCS评分和预后的关系。
结果:

Objective:Electroencephalogram(EEG) is one of nsitive indexes reflecting variations of brain fun
ction, and it is operated easily by bed to provide reliable information for treating cerebrovascular dia patients and judging prognosis. EEG monitoring has an very important clinic significance for evaluating brain function of grave cerebrovascular dia patients. To analyze variation regularrity of EEG, evaluate brain function of grave cerebrovascular dia patients, we practice long-term electroencephalogram monitoring to grave cerebrovascular dia patients in purpo to provid clinic according evaluating brain function of grave cerebrovascular dia patients.
Methods:One hundred and eighty cas with the grave cerebrovascular dia : invasion within 72h and glasgow coma score(GCS)<8 score, in Neurology Department of Chengde Medical College Affiliated Hospital from January of 2007 to December of 2009. We u Solar 2000N nerve center guardianship instrument to monitor all patients EEG, carry out relative EEG guarding and GCS grading, and calculate living number and death number of all patients. Compared with EEG grading constituent ratio of brain infarct group with brain hemorrhage group; analyzed EEG configuration feature; analyzed relationship of EEG grading and brain hernia incidence rate; analyzed relationship of GCS and prognosis.

1.EEG grading constituent ratio of brain infarct group with brain hemorrhage group on admission had no obvious significance.
2.EEG of all patients on admission were abnormal, aberration was more than 100%. Diffu slow-ware activity, eleptiform activity and down-ware amplitude were the most common form in EEG configuration of all patients;
The incidence rate of non convulsive izures in all patients was 17.2%, convulsive izures 2.2%, they had obvious significance.
3.The brain hernia incidence rate in A group patients was 17.6%, B group 31.0%, C group 47.5%, they had obvious significance in each group.
