障碍滑雪目的:利用CT灌注(Computed tomography perfusion,CTP)、CT血管成像(Computed tomography angiography ,CTA)、简易精神状态量表(M ini-Mental State Examination,MMSE)、蒙特利尔认知评估量表(Montre al Cognitive Asssment,MOCA)评价外科颞浅(STA)-大脑中动脉(MC A)搭桥术对成人烟雾病患者短期脑血流动力学及认知功能的影响。
方法:选取30例经DSA确诊为烟雾病的住院患者作为观察对象。所有患者均行单侧STA-MCA搭桥术,并于术前及术后一周行DSA、CTP 及CTA等影像学检查;测量术侧大脑中动脉(MCA)分布区和对侧镜像对照区的灌注参数,包括脑血流量(CBF)、脑血容量(CBV)、达峰时间(TTP)和平均通过时间(MTT),得到了相对CTP值(rCBV、rCBF、rTTP、rMTT)。并在术前一周和术后3个月进行MOCA、MMSE两项神经认知功能测试。
结果:
1.30例成人烟雾病患者术后CTP示脑血流均有不同程度改善,搭桥术后手术侧大脑中动脉分布区与对侧镜像区的rCBF、rCBV高于术前,rMTT、rTTP低于术前,P<0.05。所有患者术后CTA均显示桥血管吻合口形态良好,分支血管通畅、充盈良好,管壁光滑。
2.30例成人烟雾病患者颞浅-大脑中动脉搭桥术术后三个月MOCA及MMSE评分均较术前有所改善,P<0.05。

ABSTRACT

Methods:Thirty inpatients diagnod with moyamoya dia by DS A were lected as obrvation objects. All patients underwent unilateral  STA-MCA bypass surgery, and DSA, CTP, and CTA imaging examina tions were performed before and after one week  of the operation; the perfusion parameters of the middle cerebral artery (MCA) distribution ar ea on the surgical side and the contralateral mirror control area were m easured ,Including cerebral blood flow (CBF), cerebral blood volume (C BV), time to peak  (TTP) and mean transit time (MTT), the relative C TP values (rCBV, rCBF, rTTP, rMTT) were obtained. Two neurocogniti ve tests of MoCA and MMSE were performed one week before and 3 months after surgery.

1. Surgical superficial temporal artery(STA)-middle cerebral artery (MCA) bypass surgery can improve short-term cerebral he-modynamics i n adults with moyamoya dia;
2. CTP combined with CTA can effectively guide the surgical methods of adult moyamoya dia, evaluate the changes of postoperative cerebral perfusion status and the patency of bridge vesls;
3. Surgical superficial temporal artery(STA)-middle cerebral artery (MCA) bypass surgery can improve short-term cognitive function in adults with moyamoya dia.
