四种不同方法检测梅毒螺旋体抗体的比较

摘要
梅毒(syphilis)是危害人类健康较为严重的性传播疾病之一,近年来在我国的发病率呈上升趋势,梅毒的早期诊断和早期治疗对控制其蔓延至关重要。病原学检测结果的敏感性和准确性,可直接影响梅毒的治疗与预防。至今还未建立梅毒螺旋体的培养方法,直接检测病原体很困难,血清学检测在梅毒的诊断及鉴别诊断中具有决定性意义。梅毒病原体是梅毒螺旋体(treponemalpallidum,TP),其含有表面抗原,能刺激机体产生两类抗体,一类为非特异性抗体;另一类为抗螺旋体的特异性抗体。血清学试验包括非特异性试验和特异性试验。非特异性试验是以类脂质为抗原的实验方法,其中较为常用的是甲苯胺红不加热血清反应素试验(toluidineredunheatedsel'Limregaintest,TRUST);特异性试验是以梅毒螺旋体本身或非致病的螺旋体为抗原,以检查病人血清中抗螺旋体的特异性抗体,目前应用的方法有:(1)梅毒螺旋体血
球凝集试验(treponemalpallidumhemagglutinationassay,TPHA);(2)梅毒螺旋体酶联免疫吸附实验(treponemalpallidum-enzymelinkedimmnosorbentassay,TP—ELISA);(3)荧光密螺旋体抗体吸收试验(fluoresenttreponemalantibody.absorptiontest,FTA.ABS)等。通过对这几种方法的研究,寻求一种敏感性高、特异性强且更为简单快速的方法,用于输血前和对梅毒早诊断、早治疗的最佳方法。
1.研究对象和方法

梅毒28例;非梅毒对照组98例,抗核抗体(ANA)阳性患者22例,妊娠妇女26例,孕期为26.28周,健康对照组50例。
本研究利用TRUST、TPHA、TP—ELISA和FTA.ABS四种方法对上述177例标本血清进行检测和结果评价,以寻求一种敏感的、特异的、快速的、价廉的和简单的诊断方法,对梅毒进行早诊断、早治疗及早预防有着重要意义。
2.结果
err在98例非梅毒患者中,其中抗核抗体(antinuclearantibodyANA)阳性患者中,TRUST方法假阳性率最高为22.73%,FTA—ABS方法假阳性率为9.09%,TPHA与ELISA方法假阳性率最低均为4.55%。在妊娠妇女和健康正常人中,四种方法都没有假阳性结果。
在梅毒患者中,I期患者TRUST方法阳性检出率最低为77.78%,TPHA和ELISA方法阳性检出率一致为96.3%,FTA—ABS方法的阳性检出率低于TPHA和ELISA方法为85.19%。II期梅毒患者中,四种方法检出率均一致为100%,潜伏梅毒TRUST方法阳性检出率最低为85.71%,ELISA和FTA—ABS方法阳性检出率均为100%,高于TPHA方法的96.43%。
本文用四种方法对177例梅毒和非梅毒血清标本的检测,所得出的敏感度和特异性的结果,基本与文献报道相符合。
3.结论
ELTSA方法是一种新的梅毒诊断血清学方法。本文测定梅毒螺旋体感染的敏感度和特异性分别为
98.73%、98.98%.四种方法中其敏感度和特异性最高,并且该方法检测操作简便,能自动化,成本低,是一种集筛查和确诊为一体的检测梅毒螺旋体的方法。
