属于固态脉冲式的一种激光,目前在临床上对于前列
腺增生症治疗的应用最为广泛,其次为等离子,并且使

差异。钬激光对于体积增生较大的前列腺具有更好的

疗效[1-2],能够被机体快速的吸收分解[3],且钬激光在人体内组织内的穿透能力较弱,其深度仅有0.38mm,钬
激光还具有汽化病变组织的功能,在进行切割和剜除
的过程中能够有效的避免深部组织发生坏死,并且能够快速止血,还能在剜除术中将激光微爆破效应充分的利用[4]。而等离子技术中,主要是依靠镜鞘撬动和电切环切开产生作用[5]。对前列腺增生症的这两种剜除方式均是能够在前列腺的外科包膜面对增生腺体进行有效的剜除,从而达到手术的效果。该文就以激光和等离子技术在经尿道前列腺剜除术中的临床疗效进行了综述,具体如下。
1适应证的研究
在一般情况下,当老年患者年龄>70岁时或者合并伴有一种及以上的重要系统、脏器功能障碍及严重病变的前列腺增生症患者[6],在临床中属于前列腺增生症

[摘要]前列腺增生症的发病率正在呈逐年上升的趋势。经尿道前列腺电切术是目前在国际上被公认的前列腺增生治疗的金标准,但该种治疗方式治疗后的并发症发生率较高,如术后出血、电切综合征等。并且并发症的发生率与前列腺增生的体积大小密切相关,体积越大则并发症发生率越高。而经尿道前列腺双极等离子电切术是采用由等离子技术发展而形成,不仅有效降低了手术并发症发生率,且治疗效果也较为理想。经尿道前列腺等离子剜除术正是近年来在经尿道前列腺双极等离子电切术的基础上进一步发展而形成的剜除电切术,不仅切除的更加彻底,还能更进一步地降低并发症的发生率。随着大功率的钬激光仪器的引进,经尿道钬激光剜除术也逐渐的被应用于临床,并且有取代经尿道前列腺电切术的趋势。但是激光和等离子技术在经尿道前列腺剜除术中孰强孰劣,目前还未明确。

[Abstract]The incidence of benign prostatic hyperplasia is on the ri.Transurethral rection of the prostate is currently recognized as the gold standard for the treatment of benign prostatic hyperplasia in the world.But the incidence of compli⁃cations after treatment,such as postoperative bleeding,electric cut syndrome,is high.And the incidence of complications is cloly related to the size of prostate hyperplasia.The larger the volume,the higher the incidence of complications. Transurethral prostate bipolar plasma rection is formed by the development of plasma technology,which not only effective⁃ly reduces the incidence of surgical complication,but also has a satisfactory therapeutic effect.The technology of transurethral prostate plasma removal is bad on the development prostate bipolar plasma electrotomy.It could not only remove more thoroughly,but also reduce the complications.With the introduction of high-power holmium lar instruments, transurethral holmium lar ablation is gradually being applied clinically,which has a tendency to replace transurethral re⁃ction of the prostate.However,which of the lar and plasma techniques is more appropriate in transurethral rection of the prostate is not clear.
