摘要  目的:探究CT靶扫描及三维重建在早期肺癌诊断中的应用。方法:回顾性分析2018年12月至2020年5月来我院行胸腔镜下肺叶切除术且病理确诊的43例早期肺癌患者,术前采用常规扫描、CT靶扫描及三维重建进行诊断,探究其在早期肺癌诊断中的应用。结果:常规扫描与CT靶扫描及三维重建在纯磨玻璃结节、混合密度结节、纯实性结节、细支气管征检出例数上无明显差异(P>0.05);CT靶扫描及三维重建在分叶征、棘突征、胸膜凹陷征、毛刺征、血管集束征方面检出例数较常规扫描高(P<0.05)。结论:CT靶扫描及三维重建对早期肺癌的结节征象具有较高检出率,可作为早期肺癌诊断的有效方法。

Abstract  Objective: To explore the application of CT targeted scan and three-dimensional reconstruction in the diagnosis of early lung cancer.  Methods:  A total of 43 patients who were pathologically confirmed with early lung cancer and underwent thoracoscopic lung lobectomy in the hospital between December 2018 and May 2020 were retrospectively analyzed. Before surgery, they were diagnod by routine scan, CT targeted scan and three-dimensional reconstruction. Their application in the diagnosis of early lung cancer was explored. Results:  There was no significant difference between routine scan and CT targeted scan, and between routine scan and three-dimensional reconstruction in number of cas with pure ground-glass nodules, mixed density nodules, pure solid nodules and bronchiole signs (P>0.05). The detected number of cas with lobulation sign, spinous process sign, pleural depression sign, burr sign and vascular convergence sign by CT targeted scan and three-dimensional reconstruction was more than that by routine scan (P<0.05).  Conclusion: CT targeted scan and three-dimensional reconstruction are of high detection rates for nodules signs of early lung cancer, which can be applied as effective diagnosis methods for early lung cancer.

肺癌是我国死亡率最高的癌症,中晚期5年生存率约为15%,严重影响患者的健康,多数患者就诊时就已错失手术时机,临床上早发现、早诊断对患者的治疗及预后有重要意义[1]。早期患者通常无明显症状,CT是诊断早期肺癌的重要手段,而CT靶扫描技术具有层厚薄、像素小等优点,缩小了采集范围,能提高感兴趣区空间分辨率,从而精细显示病灶内部结构及边缘征象,提高病变诊断准确性[2-3]。同时其具有较快的图像处理功能,三维重建后能够清晰显示微小病灶的解剖形态,帮助医师多角度观察病灶,了解病灶构造及周围组织结构,从而提高早期肺癌确诊率[4]。因而本文采用CT靶扫描及三维重建诊断早期肺癌患
