【摘要】 目的 探究加速康复外科（enhanced recovery after surgery ，ERAS ）在达芬奇机器人辅助腹腔镜老年前列腺癌根治术中的临床应用价值。方法 选取2019年1月至2019年12月江苏省人民医院收治的96例老年前列腺癌患者为研究对象。采用随机数字表法将其分为观察组和对照组，每组各48例，均行达芬奇机器人辅助腹腔镜前列腺癌根治术，对照组患者予常规围术期干预，观察组患者予基于ERAS 理念的围术期干预。比较两组患者手术情况、恢复情况、并发症发生情况和术后日常生活活动（activities of daily living ，ADL ）量表评分。结果 两组患者手术时间、术中出血量、淋巴结清扫率比较差异均无统计学意义（均P ＞0.05）。观察组患者术后肛门排气时间、下床活动时间、住院天数均显著短于对照组（均P ＜0.05），术后并发症发生率显著低于对照组（P ＜0.05）。两组患者术后4 d ADL 量表评分均显著高于本组术后2 d （均P ＜0.05），观察组患者术后2 d 和4 d ADL 量表评分均显著高于同期对照组（均P ＜0.05）。结论 基于ERAS 理念的围术期干预能够缩短达芬奇机器人辅助腹腔镜老年前列腺癌根治术后患者的恢复时间，促进患者早期下床活动，降低并发症发生率，改善患者ADL 能力。

【Abstract 】 Objective To explore the application value of enhanced recovery after surgery (ERAS) in elderly patients with prostate cancer after laparoscopic radical prostatectomy assisted by Da Vinci robot. Method Ninety-six elderly patients with prostate cancer admitted to Jiangsu Provincial People's Hospital from January to December 2019 were randomly divided into obrvation group and control group, with 48 cas in each group. All the patients underwent laparoscopic radical prostatectomy assisted by Da Vinci robot. Patients in control group were given routine perioperative intervention, while patients in obrvation group were given the perioperative management bad on ERAS concept. The surgery condition, recovery condition, complication incidence and the activities of daily living (ADL) scale score after surgery were compared between the two groups. Result There were no significant differences in operation time, bleeding volume or lymph node disction rate between the two groups (all P ＞0.05). The postoperative anal exhaust time, ambulation time, and hospital day in obrvation group were significantly shorter than tho in control group (all P ＜0.05), and the incidence of postoperative complications was significantly lower than that in control group (all P ＜0.05). The ADL scale scores of the two groups on 4 days after surg
ery were significantly higher than tho of the same group on 2 days after surgery (all P ＜0.05), and the ADL scale score of obrvation group on the 2 days and 4 days after surgery were significantly higher than tho of control group at the same period (all P ＜0.05). Conclusion Perioperative management bad on ERAS intervention can shorten the recovery time of elderly patients after laparoscopic radical prostatectomy assisted by Da Vinci robot, promote early activity, reduce the incidence of complications and improve the ability of ADL.
