无创机械通气治疗慢阻肺合并呼吸衰竭患者的临床疗效

摘要:目的 观察无创机械通气在慢性阻塞性肺疾病(慢阻肺)合并呼吸衰竭治疗中的应用效果。方法 选取茂名市电白区人民医院2018年2月至2019年3月收治的慢阻肺合并呼吸衰竭患者115例,基于随机数字表法分为对照组(57例,有创机械通气)和观察组(58例,无创机械通气),对两组患者的治疗效果
、血气指标、住院时间、通气时间和并发症发生状况等进行比较。结果 观察组患者治疗有效率96.55%,高于对照组的84.21%,并发症发生率6.89%,低于对照组的21.05%,差异均有统计学意义(P <0.05)。治疗后,观察组PaO 2和PaCO 2均优于对照组,差异有统计学意义(P <0.01);观察组住院时间和通气时间均少于对照组,差异有统计学意义(P <0.01)。结论 予以慢阻肺合并呼吸衰竭患者无创机械通气治疗,不仅可以减轻呼吸困难及面部发绀等临床病症,改善血气指标,同时可避免并发症的发生,缩短治疗时间和通气时间。关键词:

e admitted to Maoming Dianbai District People's Hospital from February 2018 to March 2019 were lected. Bad on the parallel control method, they were divided into the control group (57 cas, invasive mechanical ventilation) and the obrvation group (58 cas, non-invasive mechanical ventilation). The therapeutic effect, blood gas index, length of stay, ventilation time and complications of the two groups were compared. Results The effective rate of the obrvation group was 96.55%, higher than 84.21% of the control group, and the complication rate was 6.89%, lower than 21.05% of the control group, with statistical significance (P <0.05). After treatment, PaO 2 and PaCO 2 of the obrvation group were better than tho of the control group, the difference was statistically significant (P <0.01). The hospitalization time and ventilation time of the obrvation group were shorter than tho of the control group, and the difference was statistically significant (P <0.01). Conclusion The noninvasive mechanical ventilation for patients with COPD and respiratory failure can not only reduce the clinical symptoms such as dyspnea and facial cyanosis, improve the blood gas index, but also avoid the occurrence of complications, shorten the treatment time and ventilation time.

金牛座男明星乡下老房子改造装修“慢阻肺”全称慢性阻塞性肺疾病(chronic obstructive pulmonary dia ,COPD ),其是临床常见慢性呼吸系统疾病之一,虽然病情发展较为缓慢,但会对患者日常劳动能力及生活质量造成较严重的影响[1],且严重的慢阻肺是造成呼吸衰竭的主要因素,所以,呼吸衰竭是慢阻肺常见的一种并发症。临床针对慢阻肺合并呼吸衰竭疾病的治疗,往往采取呼吸兴奋剂和持续低流量鼻导管给氧等方法,针对重症患者行机械通气,尽管该方法治疗效果明显,但是该方法具有较高的并发症发生率、创伤明显、患者经济压力大等[2]。近年,随着医疗行业的发展,机械通气技术不断提高,进而出现了无创机械通气治疗方法,该方法不仅创伤小,而且不会显著影响血气指标[3]。本文针对慢阻肺合并呼吸衰竭患者采取无创机械通气治疗,对效果进行综合分析,结果如下。
1 资料与方法
1.1 一般资料
选取2018年2月至2019年3月茂名市电白区人民医
院收治的慢阻肺合并呼吸衰竭患者115例,采用随机数字表法将其分为对照组与观察组,对照组57例,
其中男性30例,女性27例;年龄56~79岁,平均年龄(67.85±3.64)岁;急性生理学及慢性健康状况(APPACHE )评分10~30分,平均评分(15.69±4.31)分。观察组58例,其中男性32例,女性26例;年龄54~80岁,平均年龄(68.02±3.57)岁;APPACHE 评分10~29分,平均评分(15.71±4.28)分,两组患者一般资料比较,差异无统计学意义(P >0.05)。本研究经医院伦理委员会批准。
