摘要目的:探讨3【)打印辅助下人工股骨头置换治疗老年股骨粗隆间粉碎性骨折的效果。方法:回顾性分析2015年2月至2018年2月间南京市第一医院骨科收治的84例老年股骨粗隆间粉碎性骨折患者资料。按照随机数字表法将患者分至观察组(/7-39)及对照组(/7=45)。观察组采用3D打印辅助人工股骨头置换治疗,对照组采用股骨近端防旋髓内钉(PFNA)进行临床处理。观察并比较两组患者围术期指标及术后并发症情况。结果:两组患者治疗过程中手术时间分别为(2.32土0.63)h、(2.59±0.70)h,差异无统计学意义(&0.063);观察组术中出血量(191.37±38.31)mL>住院时间(17.59±3.19)d和术后首次负重时间(23.11±3.77)d均低于对照组相应指标水平,两组差异具有统计学意义(P二0.017、0.001、0.000)o随访14个月,末次随访时观察组Harris评分的优良率高达87.2%(44/49),对照组为66.7% (38/52),前者明显高于后者(A0.028):术后末次随访,两组发生下肢静脉血栓形成、假体松动或内固定失效、感染等并发症事件无明显统计学差异(A0.267)。结论:3D打印指导分型辅助下钢丝捆扎固定加长柄人工关节股骨头置换术后恢复较快,可明显提高號关节Harris评分且具有良好安全性,值得临床推广。

There were significant difterences between the two groups(/-^=().017,().()()1,().()()()).After14months of follow—up,at the last followup,the excellent rate of Harris score was87.2%(44/49)in the obrvarion group and66.7%(38/52)in the control group.The foniier was significantly higher than the latter(^=0.028).At the last follow•—up,there was no significant diflerence in complications such as lower extremity venous thrombosis,prosthesis looning or internal fixation failure,infection between the two groups0=().267).Conclusion: With the help of3D printing,the recovery of long handle artificial joint replacement with steel wire binding and fixation is faster,which can obviously improve the Harris score of hip joint and has g«od safety.It is worthy of clinical pri^motion.
