Obrvations.
1. In port and during cargo operation designated ‘Smoke Room’ as impost by Master was not obrved. Crews and shore personnel were en smoking in CCR and ECR which were not designated smoke area. Designated ‘Smoke Room’ does not have ‘safety matches or car type lighter’ and does not have ‘clo-top’ ash tray.
2. Crew were found to carry cigarettes and ‘HIGH RISK’ mechanical lighters.
3. Save-all(s) around main deck were found to have water, rags and plastic bottles. Plugs in all the save-all were ized.
4. Emergency shower and eye-wash room just forward of accommodation was ud as store room.

7. Oil Pollution pumps and oil pollution containers (drums with enclo top) were placed (bolted to a mounting) at the after end of main deck. Earth wires to connect the pump to ship structure were not available.
8. Foam/Fire line isolation valves were not indicated by sign-board.
9. ‘Visitor’ warning sign was not placed at gangway.
10. Visitor access control was not done at the immediate vicinity of gangway access to ship.
11. P/V valves to be tested and record available for inspection.
12. SMPEP equipment lockers near to mid-ship crane must not be lashed during cargo operations. Equipment inventory must be posted and available.
13. Pressure gauges on the outboard side of the manifolds were not available. (Gauges to be placed on the outboard side after the manifold valves to detect valves leakage).

21. Spare flanges and U-piece on deck were not properly cured to prevent shifting.
22. Sample-locker near mid-ship crane to be free from other stores and equipment. Samples to be placed orderly in cell compartment. All plastic bags to be removed.
23. MSDS for chemical stored on deck was available and placed next to the chemical drums.
24. Gangway ‘design-load’ not marked on gangway.
25. Lifebuoy 30M line not in good order, end of line fray, and not properly and correctly
cured to buoy.
26. Pump room vent trunk, gas detector sampling pipe to be clearly marked.
27. Spare Impeller for pump-room fan to be clearly marked.
28. Rescue Stretcher for pump-room was not available.
29. All valves in pump-room to be clearly marked.
30. B/A ts at the bottom of pump-room are not in accordance with FFA/LSA plan. Equipment to be removed and properly maintained and stored.
31. Water with oily sheen was en in pump-room bilges. Pump-room bilges must be dry and clean.
32. Pump-room bilge alarm to be tested and record available for inspection.
33. Pump-room bottom level gas detector to be tested and record made available for inspection. C/E to ensure span gas is available for the test.
34. Mimic diagrams for cargo lines and ballast lines to be available and clearly posted.
35. Lifebuoy lights to be of lf-igniting type (VIQ 5.53). Flashing lights are not compulsory.
36. Lights in battery room not working.
37. PPE in battery room not in good order. Storage box not marked, inventory not available.
