【摘要】目的总结腹腔镜胆总管切开探查取石术患者行一期缝合与留置T 管引流术后的效鱿鱼丝做法
果，为临床护理提供经验借鉴。方法选取2017年7月至2019年12月期间本院采用腹腔镜胆总管切开探查
取石术患者74例，根据手术方式分为一期缝合组36例和留置T 管组38例。针对围手术期患者生理心理特点及相关并发症，总结相关护理经验；比较并分析一期缝合与留置T 管对临床护理的影响。结果一期缝合组住院总费用、护理费用、术后住院时间均优于留置T 管组，差异均有统计学意义（均P ＜0.05）；两组患者CVC 、并发症、残余结石比较，差异均无统计学意义（均P ＞0.05）；一期缝合组术后1d 疼痛评分优于留置T 管组，差异有统计学意义（P ＜0.05）；两组患者首次肛门排气、首次排便以及尿管拔除时间比较，差异均无统计学意义（均P ＞0.05）；两组并发症发生率比较，差异无统计学意义（P ＞0.05）。结论腹腔镜胆总管切开探查取石术采用一期缝合与留置T 管对比，患者具有术后疼痛较轻、住院时间较短、住院费用较低及护士工作量较轻等优点。

【Abstract 】Objective To summarize the effect of primary closure and T-tube drainage in
laparoscopic common bile duct exploration,and to provide experiences for clinical nursing.Methods From July,2017to December,2019,74patients,including 36males and 38females,who underwent laparoscopic common bile duct exploration were divided into a primary closure group (n =36)and a T-tube drainage group (n =38).According to the physiological and psychological characteristics and related complications of the peri-operative patients,the relevant nursing experiences were summarized.The effects of primary closure and T-tube drainage on clinical nursing were compared and analyzed.Results The hospitalization cost,nursing cost,and postoperative hospital stay were better in the primary closure group than in T-tube drainage group (all P ＜0.05).There were no statistical differences in the incidence of CVC and residual stones between the two groups (all P ＞0.05).One day after the operation,the pain score was better in the DOI ：10.3760/cma.j.issn.1007-1245.2020.22.048
