多黏菌素E 治疗多重耐药革兰阴性杆菌重症感染10例

摘要: 目的 评价多黏菌素E 静脉制剂治疗多重耐药革兰阴性杆菌严重感染的疗效和不良反应。方法 对我院外科重症监护病房2006年5月至2007年9月收治的10例多重耐药(MDR )革兰阴性杆菌严重感染患者，在其他抗生素治疗均无效和药敏试验显示这些耐药菌仅对多黏菌素B 敏感的前提下，选用多黏菌素E 静脉制剂治疗，观察和评估其临床疗效、细菌学疗效和不良反应。结果 本组10例患者病原菌分别为铜绿假单胞菌、鲍曼不动杆菌和阴沟肠杆菌，应用多黏菌素E 治疗12例次，达到较理想的临床有效率(75.0%)和细菌学清除率(73.1%)。未发生药物相关的肾毒性、神经毒性反应或其他不良反应。结论 对MDR 革兰阴性杆菌严重感染的患者，在其他抗菌药物治疗无效时，多黏菌素E 是敏感和有效的治疗药物，但仍需注意其不良反应。

多重耐药(mult i 2drug resi stant ,MDR )菌是指一种细菌对2种或2种以上类别抗菌药物产生耐药性[1]。近年来随着广谱抗生素的广泛应用,MDR 菌不断滋生，其中以MD R 革兰阴性杆菌感染为主，最常见的有铜绿假单胞菌、鲍曼不动杆菌、肺炎克雷

这些耐药菌治疗的首选，但其敏感性和临床疗效正在逐年下降，并出现了较多耐碳青霉烯类细菌感染的报道。有资料显示铜绿假单胞菌和鲍曼不动杆菌对碳青霉烯类和氟喹诺酮类抗菌药物的耐药性在过去5年中平均增加了30%，而多黏菌素对这些耐药菌仍保持着96%以上的敏感率(MIC <4mg/L 为敏感)[223]。在新的有效抗生素研发迟滞的情况下，国外学者开始应用多黏菌素静脉制剂治疗MDR 和(或)耐碳青霉烯类的革兰阴性菌感染，并取得较好的疗效。现将我科应用多黏菌素治疗MDR 革兰营销思路八大要点
