一、预防PONV的步骤

1．判断病人是否存在PONV的高危因素

2．对于高危病人，避免可能诱发PONV的因素

3．多种方法联合应用，减少PONV的发生率

4．评估治疗效果，并采取进一步对策

二、判断病人是否存在PONV的高危因素

麻醉因素	病人因素	外科因素
吸入麻醉药氧化亚氮术后使用阿片类药物大剂量新斯的明	女性不吸烟既往PONV或晕动病史术后疼痛高度紧张和焦虑	冗长的外科手术特殊的外科手术类型 1.腹部手术 2.腹腔镜手术 3.乳房手术 4.ENT手术 5.小学暑假斜视手术

三、对于高危病人，避免可能诱发PONV的因素。可以考虑选择以下措施，以避免PONV的发生

1.选择局部麻醉（包括区域阻滞和椎管内麻醉）

2.选择propofol麻醉诱导或TIVA。避免依托咪酯诱导

3.术中避免使用氧化亚氮，可以吸入高浓度氧（FiO清雨2 80%）

4.保持充足的血容量，避免脱水

5.避免使用强效吸入麻醉药

6.术后镇痛减少或避免使用阿片类药物，可以尽量使用局部麻醉药（如椎管内镇痛）、NASIDs等

7.避免使用新斯的明

8.术中放置胃管减压
