小学教育质量综合评价方案及细则
一、指导思想
根据教育部《关于积极推进中小学评价与考试制度改革的通知》和江苏省《关
于进一步规范中小学办学行为深入实施素质教育的意见》精神，建立体现素质教育宗旨，符合轻负担高质量要求的综合评价制度，引导教师树立正确的教育观、质量观、教师观、学生观，深入践行“名在减负、名在特长、名在名师、名在文化”的办学理念，切实规范办学行为，扎实推进素质教育，全面提高教育质量，努力实现全区小学教育高位均衡发展。

分办学行为、德育工作、师资队伍、学业水平、学生特长、办学特色等六大
项，总分为100分。
三、评价标准
1(办学行为(10分)
(1)开齐规定课程，开足规定课时，不随意增减课程课时。课表、课外活动安
排表上墙上网，没有弄虚作假现象。(2分)
(2)严格执行全市统一制定的学生作息时间，确保学生每天在校集中教学时间
不超过6小时。(2分)
(3)有切实的课外作业总量监控机制，一、二年级不留书面家庭作业，中高年

教书育人、管理育人、服务育人。德育队伍健全，职责明确。(2分)
(3)德育活动形式多样，校园育人氛围浓郁。(2分) (4)班主任工作扎实有力，心理健康教育有效开展。学校、社区、家庭沟通良好，形成合力。(2分)
(5)学生熟悉《小学生守则》和《中小学生日常行为规范》，行为习惯良好，
精神风貌积极向上，无随地吐痰、乱扔乱抛、乱涂乱画、大声喧哗等现象。(2分) 3(师资队
虫草养殖伍(20分)
(1)学校有师资队伍建设五年规划，教师有个人专业发展规划，目标任务明
确，措施实在达成度高。具有相应教师资格证书的比例达100%，40周岁以下的教
师全部达专科以上学历。(4分)
(2)扎实开展校本培训和校本教研，认真落实集体备课基本要求。深入开展教

(4)全面开展“有效课堂教学人人达标”活动，每年参加区级以上教育主管部
门组织的各类优质课竞赛有一定人次获奖，优课率达50%以上并逐年增加。(5分)
(5)大力开展教科研活动，结合自身教育教学实际，围绕“以生为本，减负增效”开展课题研究，每年有一定数量的教育教学论文发表或获奖，不断取得阶段性成果并正常结题。(5分)
4(学业水平(30分)
牢固确立“让每一个学生成人，让众多的学生成才;让每一个学生合格，让更
多的学生优秀”的办学理念，区直小学、中心小学全科及格率达98%，优秀率达50%以上;村小学全科及格率达95%，优秀率达35%以上。学业水平评估由六年级语
数外质量调研(15分)、二,五年级语数外随机抽测(10分)和全区小学生常技科检测(5分)等三个部分组成。
5(学生特长(20分)
所有的学生都会演奏一种乐器，掌握两项以上运动技能，在美术、民间传统文
