ETL LISTING, LABELING, AND FOLLOW-UP SERVICE
A. General Information
Follow-up Service is unannounced product inspections for a client (applicant) at the point of manufacturer and at the point of ETL labeling to assure that the listed products are manufactured in accordance with the requirements of the listing reports. Follow-up Service is established for each applicant and manufacturing combination. Continued Follow-up Service activity allows the ETL Listing Mark to be applied to eligible products. The appearance of the ETL Listed Mark on the product is the only means to indicate that the product is being manufactured under the terms and conditions of the Intertek Testing Services’ (Intertek) Certification Agreement.
Prior to you being issued an Authorization to Mark Certificate (ATM), the following requirements must be satisfied: 1. Certification Agreement - Both copies must be signed by a Corporate Officer of your org

2. Listing Report must be issued by the Intertek Engineering Office, indicating eligibility for Listing.五世其昌
3. Client Information Sheet - Complete address (including postal code) of all manufacturing locations where
Intertek Follow-up Service inspection and labeling will occur must be submitted.
4. Initial Factory Asssment (IFA) must be conducted at each manufacturing location and satisfactorily
completed. This asssment includes a complete review of the listed product from production and of the manufacturing facility to assure that quality control, asmbly, test procedures, and records are adequate for producing and labeling an ETL listed product. This asssment is scheduled after the listing report is issued and notification from the applicant regarding production schedule is received. However, the IFA may be performed prior to the issuance of the listing report with the first product inspection performed when this report is issued.
a. The Authorization to Mark Certificate (ATM) is issued once the applicant/manufacturing combination
has received a factory and product inspection.
b. Initial Product Inspection (IPI) –When a new Applicant us a subcontracted manufacturer who is
already in current follow-up rvice for another Applicant’s product, an Initial Product Inspection (IPI)
will be required. Upon successful completion of the Initial Product Inspection (IPI), the Authorization to
Mark Certificate (ATM) will be issued. Intertek will maintain parate files and charge parate follow-
up rvices fees for each Applicant/Manufacturer combination.
5. Labeling - The listing mark should be placed on or near the product name plate and should be visible after

the product is installed. It can be affixed by means of a parable label purchad from Intertek or directly imprinted on the rating or nameplate of the product or device. Method of affixing the listing mark is left to the discretion of the applicant. Further information on this subject is included in this text. A facsimile of the ETL Listing Mark as it will appear on the product must be submitted to Intertek for approval, if the Direct Imprint method of labeling is to be ud. ETL labels may be applied to products only at the locations receiving Follow-up Service inspections.
6. Purcha Order / Payment - Payment to cover the cost of the first year of follow-up rvice is needed to

This should cover the initial factory asssment, three quarterly follow-up inspections, four certification fees, travel expens, and either the quantity of labels needed or the direct imprint fee. Basic follow-up rvice for United States or United States/Canadian Certification is quarterly. Basic follow-up rvice for Canadian Certification only is Semi-Annual. The frequency of inspection may be bad on the volume of production. Additional fees may apply if the inspection takes in excess of three hours to complete.

2. Labeling Fees
There are two methods available to the applicant for applying the ETL Listing Mark to the product. The first method is to purcha Separable Labels through Intertek. The cond method is to supply your nameplate, referred to as Direct Imprint including the ETL Listing Mark on the nameplate of your product.
The applicant must decide on one of the two methods, and lect inside CIS. Details are described below:
** Method 1 Separable Labels - ETL Listing Mark (purchad through Intertek)
Refer to the price list parately from Intertek ETL Labels office, the special handling Fee of 15% will be added to
label orders which are required within 3 business days, overnight delivery rvice, or other special requests. Freight
on Board - Shipping cost will be prepaid at Shipping Point and will be invoiced to client.
Shipment lead-time - within 7 to 10 business days from receipt of your order, and the purcha order is required prior
to processing order to cover labels, tax, shipping or special handling

PRODUCT GROUP ANNUAL DIRECT IMPRINT FEE

Certification, inspection, and labeling fees are per manufacturing facility. Manufacturing facility is defined as the
number of locations manufacturing a product under a parate control number.
If you wish to Direct Imprint the ETL Listing Mark to the Product's nameplate, you may download the ETL Listing
Mark from our website –/Functions/Sales-Marketing/Certification-Marks/ Note:A facsimile or email must be submitted to Intertek Follow-up Services Office for approval prior to u. The facsimile need not have a control number. A control number will be issued after signed Certification Agreements have been received by the Follow-up Services office, approval of the facsimile of your propod Listing Mark, satisfactory completion of the Listing Report, and scheduling of a factory asssment in your facility.
3. Multiple Listee Fee (when applicable)
The Applicant will be billed an annual fee of US$283 per Multiple Listee to maintain the Multiple Listee information in the Directory of Listed Products. Multiple Listee information will appear on the Correlation Page of the product Listing Report (if applicable). If Unique Multiple Listee Control Number is requested, the Applicant will be billed an annual fee of US$523 per Multiple Listee.
4. Reinstatement fees for suspensions and terminations of ETL file
The Applicant will be billed US$370 per ETL file.
5. Semi-Annual Inspection Program Review Fee
The Applicant will be billed US$525 per factory.
6. Penalty Fees – Misu and Non-Complying Product Investigations
The cost of misu of the mark or non-complying product investigations can be $2,625 or above for the first offence and $5,250 or above for the cond with the third resulting in termination of the listing.
7. Payment Information for Follow-up Service

The information required on a purcha order for Follow-up Service is as follows:
∙Company name, address, phone/fax number, bill to name, and/or bill to department
∙Purcha order number
∙Purcha order issue date and expiration date
∙Name of person responsible for renewing the purcha order

C. The ETL Listing Mark
The ETL Listing Mark consists of the following four items:
1. The ETL Certification Mark with “US” and/or “C” as country identifiers for the United States and
Canada. The letter “C” adjacent and to the lower left side of the ETL Certification Mark indicates that the
product complies with a Canadian standard. The letters “US” adjacent and to the lower right side of the
ETL Certification Mark indicates that the product complies with a US standard. The identifiers must be
legible on the product marking.
2. The word, "Listed" or "Classified" or "Recognized Component" (whichever is appropriate). The word,
"Listed" is to be incorporated into the ETL Certification Mark. If upon reduction, the word “listed” is not
黔驴技穷legible as part of the trademark, it shall also appear parately.
3. The Control Number issued by Intertek. This five to eleven digit number identifies to the manufacturing site
for each applicant.
4. A product descriptor, which refers to the national standard ud for certification shall be ud.
For United States Standards, the words, “Conforms to” shall appear with the standard number along with the word, “Standard” or “Std.”, e xample: “Conforms to ANSI/UL Std. XX.”
For Canadian Standards, the words “Certified to CAN/CSA Standard CXX No. XX.” shall be ud, or abbreviated, example: “Cert. to CAN/CSA Std. CXX No. XX.”
If the manufacturer wishes, they may u the standard title, example “Telephone Equipment.”
D. Component Retesting Fees (when applicable)
If the ETL Listing Report (CDR) contains a requirement for annual retesting of unlisted components, the fees will be quoted parated by local Component Evaluation Center (CEC).
E. ETL Follow-up inspection for Cable
To better control the final production of power supply cords and cord ts are continuously in compliance with UL 817, Intertek has implemented a new Follow-up Service program for this particular category. Effective 1 May 2010, the quarterly Follow-Up Factory Inspection will include a quarterly Sample Testing which Intertek inspectors will perform a witness test on site and annual sample pickup for testing at Intertek laboratory
