Audit / Survey Worksheet

INTRODUCTION: The ABC Corporation Supplier Quality System Survey / Worksheet is intended to rve as a basis for evaluating the potential of a supplier’s quality system. While this does not substitute for demonstrated results, it does provide a systematic method of reviewing and asssing the manner in which the supplier has organized their quality system with the purpo of choosing a supplier that will be able to meet or exceed the quality standards of the ABC Corporation.

AUDITOR QUALIFICATIONS: Individuals utilizing the Supplier Quality System Survey should have qualifications consistent with tho outlined in the most recent revision of ANSI/ASQC Q10011-2, “Guidelines for Auditing Quality Systems - Qualification Criteria for Quality System Auditors”.

ISO-9000 REGISTRATION: In keeping with ABC’s objectives to source from ISO / QS-90

00 compliant companies, approval status may be granted to companies providing a copy of a registration certificate (including scope) to the appropriate ISO/QS-9000 standard (i.e. ISO-9001, ISO-9002, ISO-9003, and QS-9000).

SCORING: The survey consists of 173 questions, scoring is as follows:

Informal system fully implemented, or partially documented system partially implemented.	1 point
Fully documented system partially implemented.	2 points
Documented system, fully implemented.	3 points
Documented system, fully implemented (includes documented measurable results and corrective actions). thames	4 points
Not Applicable (NA)	Not Scored

The supplier’s approval status is determined by dividing the raw points score by the points available for applicable questions, and using the scale as follows:

Approved	75% Required Minimum
Conditionally Approved	60% Required Minimum
Approval Withheld	Less than 60%

The scoring requirements apply to each applicable ction, as well as the survey in total. Points are distributed between ctions as shown below.
